What Medigap Doesn't Cover in the USA 2026
Did you know that even with "supplemental" insurance, you could still receive a medical bill for thousands of dollars for services like dental work or hearing aids? Many people assume that Medigap fills every single hole that Original Medicare leaves behind. These policies follow very specific rules about what they can and cannot pay for in 2026.
You should think of Medigap as a tool that helps with the costs of services Medicare already approves. It is not a separate health plan that adds new categories of care. If Medicare Part A or Part B refuses to pay for a procedure, your Medigap policy will almost always refuse to pay its portion as well.
The Basics of Medigap Support
Medigap plans exist to handle the "cost-sharing" parts of your bills - this includes the money you normally owe out of your own pocket, like deductibles, coinsurance and copayments. When you visit a doctor, Medicare pays its share first and then your supplement pays some or all of the remaining balance.
It is important to remember that Medigap does not replace your Medicare coverage. You still must pay your monthly Part B premium to keep your insurance active. If you stop paying for Medicare, your Medigap policy becomes useless because it has nothing to supplement.
Prescription Drugs & Daily Care
One of the most frequent surprises for seniors is that Medigap does not pay for your local pharmacy bills. Since 2006, new Medigap policies are prohibited from including prescription drug coverage. You must join a separate Medicare Part D plan if you want help paying for your medications.
Long-term care is another major area where you are on your own. If you need to stay in a nursing home because you are frail or need help with daily living, Medigap will not cover those costs. It also excludes private duty nursing, which is when you hire a professional to monitor you at home or in the hospital.
Common exclusions in 2026- Retail prescription drugs (Part D)
- Long-term custodial care in nursing homes
- Private-duty nursing services
- Any service that Medicare deems not medically necessary
Vision, Dental, & Hearing Gaps
Are you planning to get new glasses or a dental cleaning this year? You should know that Medigap usually excludes these "lifestyle" medical services. Because Original Medicare does not cover routine vision or dental work, your supplement stays silent on the bills as well.
This means you are responsible for the full cost of the following items
- Routine dental exams and fillings
- Eye exams for glasses or contacts
- The actual cost of frames and lenses
- Hearing helps and the exams to fit them
If these services are important to you, you might need to look for stand alone private insurance policies. Some people choose Medicare Advantage instead for these perks but remember that Advantage plans work very differently than the Medigap besides Original Medicare combination.
Annual Limits & Foreign Travel
Many Medigap plans do not have a "safety net" or a maximum amount of money you have to pay in a year - this is a unique risk you should consider. Plan K or Plan L are the exceptions to this rule. In 2026, the are the out-of-pocket limits for those specific plans
- Plan K
$8 000 yearly limit - Plan L
$4 000 yearly limit
If you travel outside the United States, you might find some relief. Some specific Medigap policies offer a benefit for foreign travel emergency care. Since Medicare usually stops at the U.S. border, this is one of the few times a supplement pays for something that Original Medicare does not cover at all.
FAQ
Does Medigap cover my monthly Part B premium?
No, you are responsible for paying your own Part B premium to the government. Medigap only helps pay for the services you receive, not the cost of the insurance itself.
Can I have a Medigap plan and a Medicare Advantage plan at the same time?
No, it is actually illegal for anyone to sell you a Medigap policy if they know you have a Medicare Advantage plan. You must choose one or the other.
Will Medigap pay for my deductibles?
Many plans cover the Part A hospital deductible - However, if you are new to Medicare, you generally cannot buy a plan that covers the Part B (doctor) deductible.
